Are parents prepared for discharge from Children’s Community Nursing Teams?

Abstract:

The aim of the study is to review the discharge process of children with acute health conditions from a children’s community nursing team (CCNT). Parent and children’s community nurse (CCN) perspectives will be examined using a mixed methods design. The researcher will discuss whether a standardised tool to aid this process can be utilised to enhance family centred care (FCC) and improve patient outcomes thus ensuring safe and effective discharge.
